THE CHALLENGE OF FRAGMENTATION
Every success, however, creates a new responsibility.
As knowledge expands, medicine becomes more specialised. As technology advances, the volume of information grows. As health systems develop, care can also become more fragmented.
This is understandable. To a large extent, it is the natural consequence of scientific progress.
But it raises a new question.
Who still holds the coherence of the whole?
Too often, that responsibility ends up with the person least prepared to carry it: the patient.
At each new consultation, the patient retells the story. Carries the tests. Tries to connect reports and decisions made in different contexts.
The patient can become the only person still holding the continuity of their own medical journey.
We do not believe this is inevitable.

Technology is indispensable. Centres of excellence are indispensable. Highly specialised expertise is indispensable.
But sophistication and understanding are not always the same thing.
No technology, institution or reputation can, on its own, replace the reasoning required to place the available information back into the singular history of a person.
A result can be accurate.
A diagnosis can be established.
A treatment can be medically justified.
And yet part of the situation may still remain insufficiently explained.
The value of medicine is not measured only by what it discovers.
It is also measured by what it allows us to understand.
Tomorrow
Medicine will continue to advance through new discoveries, more precise technologies and deeper knowledge.
But part of that progress will also depend on our ability to connect with the same rigour with which we have learned to discover.
To organise with the same discipline with which we have learned to measure.
To challenge and compare with the same precision with which we have learned to observe.
To understand with the same humility with which we have learned to know.
Information is not yet understanding.
Understanding begins when information is no longer considered in isolation, but can be placed within a history, a chronology and a coherent whole.
